On Tuesday, January 19^th, child will receive their second dose of H1N1 vaccine at  BOTH FriendshipLearningand Early Learning Centers.  This second dose is needed for children to have full protection against H1N1 disease.  Children in fourth grade will receive their vaccine during the school day without parents needing to be present.  Children in third grade and younger will receive their vaccine on the Tuesday, from 8:00 – 8:30 a.m., with a parent or designated adult being present.  Vaccines are provided at no cost. 

If your child has already received their second H1N1 vaccination, or if you do not want your child to receive this second vaccination please notify your child’s school, or contact the Fond du Lac County Health Department.  No child will be given a vaccine without a signed authorization form.
